Submit Vaccination Records

Before your dog's first day of training, we require up-to-date records for Rabies, DHPP, and Bordetella. Please upload copies of your vet certificates below.

Accepted files: PDF, JPG, or PNG. A clear photo of the vaccine certificate from your vet works perfectly.

Max file size: 10 MB. You can attach a single PDF or a photo of the certificate.
